In order to protect all, especially vulnarable and immuno-compromised patients, 

I have implemented a new safety policy:

 

Two Covid vaccines or proof of physician's certificate of recovery

from Covid are mandatory

in order to be eligible to receive treatments.

 

 

 

Effective August 23rd 2021

all patients must be fully vaccinated or provide a physician's certificate of recovery

(i.e. have a minimum of two base vaccinations or a certificate of recovery from Covid and provide legal proof.)

 

 

*Note: Using a fake vaccination card will result in a $500 office fine,

            is a federal crime and will be reported to the authorities.
